| Directions to SDHD Facilities |
1. IONKWANONSASETSI - Adolescent Group Home Located on the West end of Cornwall Island, coming off the Cornwall Bridge, take the first right at the cross roads. Next, take another right - heading towards AMS school. Prior to reaching the school, take a right at Island Pizza. The group home is the biege building at the top of the hill.
2. Community Health Representatives & Environmental Health Officer Offices Located on the West end of Cornwall Island, coming off the Cornwall bridge, take the first right at the cross roads. Next, take another right - heading towards AMS school. Entrance to the EHO office is directly across from the AMS school driveway (look for pink building).
3. KAWEHNOKE DAY CARE Located on the East end of Cornwall Island, coming off the Cornwall bridge, turn left at the cross roads. About a quater of a mile, the day care is located on your left (same bulding as Kawehnoke Community Center). You should see a ball feild in the yard as you come up the hill.
4. TSIIOKWANONHSO:TE / KAWEHNOKE MEDICAL CLINIC Located on the East end of Cornwall Island, coming off the Cornwall bridge, turn left at the cross roads. Approximately 4 miles in, there'll be a sign on your right for the clinic, and Riverview Apartments, take that right onto McCumber Rd.. Drive until you hit the paved road on your right (approx. 1 mile in), after turning, the facility is the brick building about 200 ft. on your left.
5. KANONHKWATSHERI:IO Health Care Facility Driving from both Cornwall, or Massena, take Rt. 37 (located on US side of map), and head East toward Fort Covington/Malone. From the US Customs, you will drive about 4 miles before coming to the Hogansburg traffic light. At the traffic light turn East onto St. Regis Rd.. Stay on St. Regis Rd. for approx. 2 miles until you come to where the road divides, and the you see the sign "Welcome to Kanatakon". Before this division, take your first left, stay on this street until the second intersection where you will take another left. Drive along this road until you come to a stop sign. Drive through the stop sign and you will see the Health Facility to your right. Visitor Parking is immediately to your left.
6. IAKHIHSOHTHA - Home for the Elderly Driving from either Cornwall, Massena, or Fort Covington on Rt. 37 eastbound from Cornwall & Massena, & westbound from Fort Covington. Just south of Hogansburg, where the speed limit changes back to 55 mph, there'll be a turning lane to turn east onto Cook Rd. This road is located on the north side of Gas Station #9. Take that road until you reach Y in the road. Next, turn left at the Y. Stay on that road when it vears right, rather than driving straight. You should be driving toward the River, you will also pass a sign that says "Welcome to Tsisnyeneh". Take a right onto River Rd., and drive for about 2 miles until you reach the school road, where you will take a right. You should also see a sign for Iakhihsohtha, and MCA Facilities, take that road to the very end, which will also vear right, and Iakhihsohtha is the brick building at the end of the road. 7. TSISNYENEH DAY CARE Same directions as Iakhihsohtha, the day care is located just to the right of Iakhihsohtha. |
